Endodontic Treatments

Endodontic Treatments in Oxfordshire

Dr James Fitton has completed specialist endodontic training at the prestigious Guy's Hospital in London and has travelled extensively to refine his expertise. His passion for advanced endodontics took him to MedellĂn, Colombia, where he trained under world-renowned endodontists Dr Paula Villa and Dr Felipe Restrepo. During his time there, he enhanced his skills in advanced microsurgical techniques, tooth autotransplantation, and intentional reimplantation. James is proud to offer these specialist treatments here in Oxfordshire, providing patients with world-class care closer to home.

Root Canal Treatment

At Apex Endodontics, we provide specialist root canal treatment for cases where the dental pulp has become inflamed or infected, whether through decay, trauma, or previous restorative procedures. Our goal is to preserve the natural tooth and eliminate infection with predictable, evidence-based outcomes.

Our approach includes:
  • Thorough assessment and diagnosis using advanced imaging
  • Gentle, precise instrumentation under high magnification
  • Disinfection and obturation of the root canal system with the latest materials
  • Placement of a temporary or permanent coronal seal, with restoration referred back to you if appropriate

We welcome referrals for primary root canal therapy, especially in cases involving challenging anatomy, posterior teeth, or patients requiring enhanced pain or anxiety management.

Referral benefits:

Reliable treatment outcomes using advanced equipment

Enhanced patient comfort and communication throughout

Prompt and clear reports returned to your practice

Root Canal Re-treatment

We accept referrals for root canal re-treatment when a previously treated tooth presents with persistent symptoms, radiographic signs of periapical pathology, or suboptimal obturation. Re-treatment is a technically demanding procedure that requires careful disassembly, disinfection, and identification of missed or untreated canals.

Our protocol includes:
  • Removal of existing root filling materials or posts
  • Exploration under dental operating microscopy
  • Thorough cleaning, disinfection, and re-obturation of the canal system
  • Restoration with a temporary filling or returned to you for permanent restoration

With our specialist expertise, many previously failing cases can be resolved successfully, avoiding the need for extraction or more invasive procedures.

Referral benefits:

Access to microsurgical techniques and CBCT if required

Comprehensive re-treatment planning and communication

Patient retention with clear handover post-treatment

Apicoectomy (Root-End Surgery)

We provide microsurgical apicoectomy for patients with persistent periapical pathology following previous endodontic treatment. Using operating microscopes and ultrasonic retropreparation techniques, this procedure is minimally invasive and highly effective.

Indications include:
  • Persistent periapical lesions unresponsive to non-surgical treatment
  • Anatomical complexities preventing successful orthograde re-treatment
  • Root-end pathology with associated sinus tracts or cystic lesions

Our technique involves careful resection of the apical 3mm of root, removal of infected tissue, and retrograde filling with MTA or bioceramics.

Referral benefits:

Predictable surgical outcomes with specialist care

Enhanced healing through minimally invasive techniques

Case updates and clear communication to support your treatment planning

Perforation Repair

Perforations, whether iatrogenic or pathological, can compromise tooth prognosis if not managed properly. At Apex Endodontics, we offer referral-based perforation management using high magnification and biocompatible repair materials.

Our approach includes:
  • Accurate diagnosis and imaging to locate the perforation
  • Gentle cleaning and disinfection to control infection
  • Sealing of the perforation with materials such as MTA or Biodentine
  • Follow-up imaging and monitoring to assess healing

Early referral improves outcomes, particularly in cases involving furcal or strip perforations.

Referral benefits:

Access to advanced repair techniques and materials

Tooth preservation in high-risk cases

Peace of mind knowing your patient is in expert hands

Resorption Management

We accept referrals for internal and external resorptive defects, including cases detected incidentally or presenting with symptoms. These complex cases require detailed diagnosis, often involving CBCT imaging, and may need surgical or non-surgical intervention.

Management may involve:
  • Internal resorption: orthograde access, cleaning and obturation
  • External cervical resorption: surgical debridement and restoration
  • Extensive cases: multidisciplinary planning or extraction when indicated

We tailor treatment based on the location, severity, and prognosis of the resorptive process.

Referral benefits:

Accurate diagnosis and tailored treatment options

Tooth preservation whenever possible

Specialist follow-up and long-term monitoring

Trauma Management

We work closely with referring dentists to manage dental trauma cases involving luxation, avulsion, root fractures, and pulp necrosis. Whether you require urgent support or a longer-term treatment plan, we're here to assist with endodontic aspects of care.

Our services include:
  • Immediate assessment and pulp vitality monitoring
  • Splinting, repositioning, and endodontic management
  • Treatment planning for revascularisation or apexification
  • Ongoing follow-up with radiographic monitoring

Timely referral in trauma cases can significantly influence prognosis and treatment outcomes.

Referral benefits:

Rapid access to specialist care for dental injuries

Collaborative treatment planning and shared decision-making

Long-term support to help safeguard the future of the affected dentition

Intentional Reimplantation & Autotransplantation

In select cases, we offer intentional reimplantation or autotransplantation as alternatives to extraction or prosthetic replacement. These procedures are carefully assessed and planned, often in conjunction with 3D imaging and surgical guides.

  • Intentional reimplantation: ideal for teeth requiring extra-oral endodontic treatment where conventional access is not possible.
  • Autotransplantation: particularly suitable for young patients with suitable donor teeth and favourable root development.

We provide detailed case assessments and coordinate with referring dentists and orthodontists where appropriate.

Referral benefits:

Access to niche, specialist treatment options

Preservation of the patient's natural tooth structure

A biological alternative in cases where implants are not ideal
